Supported Living and Direct Care Staff Job Description
All direct care employees of the Semi-Independent and Supported
Living Programs and must fulfill the following job requirements:
• Relate, interact, and communicate positively with the individuals.
• Support and assist the individuals in
developing skills that will further their independence, increase
their community involvement, and improve their quality of life.
• Perform medication monitoring,
ensuring all prescriptions are filled and taken according to
physician's instructions.
• Conduct fire drills according to
agency and DPW requirements.
• Adhere to all agency policies and
procedures in regards to handling individual and miscellaneous
household funds.
• Motivate and assist the individuals in
all areas of routine household maintenance and upkeep.
• Motivate and assist the individuals in
ongoing purchase and care of personal belongings.
• Motivate and assist the individuals in
preparing nutritious meals.
• Encourage and assist the individuals
in the use of available means of transportation.
• Display a positive, flexible, and
cooperative attitude toward supervisors and coworkers.
• Report or communicate all information
pertinent to the continuity of care of the individuals as well as to
routine agency operations.
• Attend all agency required meetings
and training.
• Comply with all agency and DPW
regulations and policies.
• Other duties as assigned.
The
following statements reflect the general requirements necessary to
perform the principle functions of all direct care staff and shall
not be construed as a detailed description of all the work
requirements that may be inherent in the occupation.

Supportive Living Services Direct
Care Staff General Job Description
All direct-care employees of Supportive Living Services are expected
to fulfill the following job requirements.
• Relate, interact, and communicate positively with the individuals.
• Support and assist the individuals in developing skills that will
further their independence, increase their community involvement,
and improve their quality of life.
• Administer and document medications according to agency and DPW
policies.
• Conduct fire drills according to agency and DPW requirements.
• Adhere to all agency policies and procedures in regards to handling
individual and miscellaneous house funds.
• Participate in the general upkeep of the individuals' homes and
property.
• Prepare, or assist the individuals in preparing, family style
meals.
• Provide any necessary transportation using agency vehicles and
follow all applicable policies and procedures.
• Display a positive, flexible, and cooperative attitude toward
supervisors and co-workers.
• Report or communicate all information pertinent to the continuity
of care of the individuals as well as to routine agency operations.
• Actively participate in the agency on-call system either as
starred-supervisor or the designated on-call staff.
• Attend all agency required meetings and training.
• Comply with all agency and DPW regulations and policies.

These requirements must be initially met by all direct-care staff
and therefore will not be used for evaluation purposes.
Physical requirements: The physical demands described here are
representative of those that must be met by an employee to
successfully perform the essential functions of this job. Reasonable
accommodations may be made to enable individuals with disabilities
to perform the essential functions.
While performing the duties of the job, the employee is required to
frequently climb stairs, squat, bend, stand, and walk, demonstrate
manual dexterity, speak and hear.
The employee must routinely lift and/or move up to 20 pounds,
usually waist high; reach above shoulder height; grasp and use fine
motor skills.
Clothing requirements: Shoes are to be worn on duty. Latex gloves
are to be used in accordance with the infection control policy.
Casual attire is suggested when working at the home.
Other Skills & Abilities: Ability to safely transport individuals in
a vehicle and have a valid driver's license. Ability to provide
acceptable criminal history checks.
